How to get NAPA mail-in rebate:
(1) Print out NAPA mail-in rebate form.
(2) Mail this completed form (to ensure proper processing, please write neatly) along wit NAPA sales receipt that shows the qualifying NAPA battery.
(3) NAPA receipt must be dated from March 1 through May 31, 2011.
(4) Envelope must be postmarked by July 15, 2011.
NOTE: Rebate limit is two (2) batteries per household.
